Recurrent UTI, Now Loin pain (528/1700)

A 29yo lady admitted with hx of repeated UTI now developed hematuria with loin pain. What is the most probable dx?
a. Acute pyelonephritis
b. Chronic pyelonephritis
c. UTI

Loin pain= pyelonephritis, recurrent UTI=VUR..........Chronic pyelonephritis is characterized by renal inflammation and fibrosis induced by recurrent or persistent renal infection, vesicoureteral reflux, or other causes of urinary tract obstruction.

n.b. Bladder stone would cause suprapubic pain, UTI would cause frequency/dysuria w/o loin poin, acute pyelonephritis: fever would be present
