Slow answers after fall (211/1700)

A 72 yo man fell while shopping and hurt his knee. His vitals are fine. He speaks in a low voice
and is very slow to give answers. What is the most probable dx?

a. Alzheimers
b. Vascular demetia
c. TIA
d. Pseudo-dementia
e. Picks dementia





























answer: B ... fell suddenly e.g. had a small infarct, now talking slow which is the characteristic of the disease

Let's look at the DD:

  • Alzheimers : memory impairment 
  • Lewy body dementia : hallucinations, parkinsonism
  • Picks dementia (fronto-temporal dementia): disinhibition, memory spared, talking nonsense
  • Vascular dementia : slow talking, slow thinking, slow walking, can be abrupt onset
  • Pseudo dementia of depression: depressed, self harm, suicide, orientated
  • Stroke/TIA: focal neurological deficits
