Respiratory problems after PROM (813/1700)

A child born at 36wks developed difficulty in breathing with intercostal recession and nasal
flaring. His temp is normal but his mother had PROM 48h ago. What is the most likely inv that
will lead to tx?

a. Blood culture
b. CXR
c. Stool culture
d. Sputum culture































Answer:B , Chest Xray will show pneumonia or ARDS


dx: PNEUMONIA
• consider in infants with prolonged or premature rupture of membranes, maternal fever, or if mother is GBS positive
• suspect if infant exhibits respiratory distress, temperature instability, or WBC is low, elevated, or left-shifted
• symptoms may be non-specific
CXR: hazy lung and/or distinct infiltrates (may be difficult to differentiate from RDS)
