A 54yo man has recently been dx with moderate depression. He has hx of MI and is suffering
from insomnia. What is the drug of choice for him?
a. Citalopram
b. Lofepramine
c. ECT
d. Haloperidol
e. Diazepam
answer A
pramine/triptyline = TCA contraindicated in recent MI
haloperiod/diazepam = not used for depression
ECT= excessive for moderate depression
Citalopram= it's contraindicated in those with QT interval prolongation and cautioned in pts at higher risk of developing torsades de pointes but by exlusion seems the best choice here
the best antidepressant post MI is actually sertaline
