Subacute Thyroiditis/ De Quervain's disease
  • Controversial aetiology associated with viral infection, usually follows URTI.
  • Thyroid becomes congested, swollen, has a firm irregular pattern and slightly tender. 
  • Associated with fever, malaise and neck pain. 
  • Remissions and exacerbation over a few months. Self limited course. 
  • ESR high
  • Normal or low leucocyte count 
  • Iodine uptake is depressed
  • Slight elevation of T4
  • Rapid response to prednisolone is diagnostic
